Due to our continued commitment to health and safety, we have made some changes to office protocols/procedures. 

*All forms are now online.  Please click the following links to fill out the forms prior to your appointment.

COVID Questionnaire

*When you arrive, please remain in the car and give us a call at 615-891-1243.  A team member will ask you a few questions, verify your insurance and guide you from there. 

*We ask that you do not bring along any friends or family members to the appointment. If someone does accompany you, we ask that they remain in the car for the duration of the appointment.

*Before entering the office, a team member will meet you at your vehicle to conduct a temperature check. We will then determine if the patient is cleared to come inside the office.

*We have hand sanitizer that we will ask you to use when you enter the office. You will also find some in the reception area and other places in the office for you to use as needed.


*Everyone entering MUST wear a face mask!  

*You will be sent a short COVID-19 assessment the day before your appointment.  If you have any positive symptoms or answer any of the questions positively, you will need to reschedule your appointment. You will be asked those same questions again right before you enter the office.
